NEW CLIENTS - please fill out the New Client Registration form and submit it at least 24 hours prior to your first appointment. You only need to fill this form out ONCE - depending on the clinic's operating hours and when you submit the form, you may receive additional texts requesting that you fill it out (the texts are automated). Simply reply to the text letting us know that you've already submitted the form so we can ensure it was received on our end. We'd also love for you to fill out the Patient History Form (one per pet), as this can expedite the often time-consuming initial part of the visit.

EXISTING CLIENTS - you do not need to fill out the New Client Registration form, unless there have been changes to your contact information and you'd like to submit them ahead of time. We would love for you to fill out the Patient History Form (one per pet) in order to shorten the time needed for the initial part of the visit.
